Personal Directive Creation

Often referred to as a living will, a personal directive allows you to appoint an individual to make health care and other important decisions on your behalf when you are no longer able to. In your personal directive, you can specify what kind of health care and treatment you want to receive when you lose your mental capacity to make your own decisions.

A personal directive helps keep your wishes intact and reduce uncertainty during difficult times. The need may be short-term or long-term. Your personal directive will not come into effect until your doctor says you are no longer capable.
